Generally though, there is NO self-help. You have to consult a therapist/psychatrist altogether and work on it with them and be 100% truthful to them, that is the only method I can find towards it, please trust me when I say this as there are people who think they can help themselves, then forget they were doing that, and go on auto-pilot, and the cycle continues.

Have someone to hold you accountable. Find someone you can be open with about your struggle, and have them be firm in telling you "hey, you need to start doing this thing now". Tell them how it's hard for you.
If you're struggling to start something, ask a family member to literally sit down with you to help you get started. Typically once you really get started, you can focus in on it.

I will give you three tips which can definitely help you:
Use procrastination paradox. Procrastinate by doing other tasks you’ve been putting off. If you hate doing laundry, Procrastinate on that by organizing your sock drawer. It’s like a procrastination inception. :Prayge:
The second thing is is, you can hire a clone. Let your clone handle the boring stuff while you can focus on the important task like playing valorant. :WICKED:
The third one is my personal favorite. Build a time machine, go back to yesterday, and tell your past self to get the work done. That way the present you does not need to do anything today. :POGGIES:

I am literally going through your problem, and eventually I figured this out for myself, and then as I got into the rythym of being productive, since I finally felt good about myself, it's become easier to start things.

if u can't get that, I've found that setting my self a time limit for tasks helps, I never have the motivation to do anything if it's not nearing a deadline, but it doesn't always work
Do you need prescription for this
You can try to complete any activity along with other peolple like friends/family. For instance if you wanna study, you can study in a group or if you wanna clean your room ask someone to accompany you. So I haven't actually tried this since I don't have any friends and have like 1 family member but you can give it a shot.
