headphones always, because using in ears can produce Otitis or more cerum, even if you are extremely clean with your hearing aids, it does not prevent it from happening to you. Besides, the ear needs air to be circulating, and at least with headphones, this can happen, although partially, but it is still better than the lack of air circulation caused by in-ears.
So if you want to use in ears it would be at least 2 hours, for long days always headphones. (It is a myth that the headband of the headphones cause hair loss or affect the shape of the head, although the skin yields by prolonged weight adapting, in 10 minutes returns to normal.

So many reasons why they are better. Only issue is that if they don't fit into your ears it can be shitty trying to find right eartips.
