I asked a similar question to pansy when she streamed the other day. She said casters wont get fined for accidental miss-pronoun.
i dont think players get fined for what they say in comms . A lot of people swear which is finable as a caster so yea

I don't think fine is enough. The best solution is to declare defeat for that team on that map or series and ban the player who said it.
I'm p sure its fine according some casters who have spoken about it. Accidentally misgendering isn't really a problem. If anyone says otherwise they are relying on a strawman argument.
