don't let this fool you, Sen are an exception but Riot is releasing insane patches which shake things up too much which allows an unstable landscape where lower teams can capitalize on. it makes for good upsets but how are you going to establish a loyal fanbase if everyone is losing? sure you want the game to thrive and not go stale but some changes are too drastic and its what i liked about CS before where some teams were really good and when an upset happened, it was rare and felt good.

I think it will be difficult for a team to remain on top after every patch, only sentinels perhaps.

Adapting quickly to changes in the game whether big or small is also any teams job, goes for both established and "lower teams".
